Are you deaf? deafened? have a hearing loss?

Do you speak English? French? German or another language?

Can you speak at all?

Are you fluent in ASL? BSL? another sign language?

Were you born deaf? Or did you acquire hearing loss or deafness later?

Is  your hearing loss inherited? Due to infection or noise or  one or more of a large number of other reasons?

Do you hear anything at all?

Can you sense vibrations? Touch the vibration?

Do you hear low frequencies? High frequencies?

Do you have a “cookie cutter” shape loss which makes it impossible to understand “human speech” frequencies?

Do you hear male voices better than female voices?

Do you hear music ? Or does it sound like one note noise? Did you ever hear music? Do you miss it so much?

Do you use hearing aids, listening devices (many kinds), loops, lipreading or lipspeakers?

Do  you feel alone and lonely also at times?

Do you need captions?
